Your First Visit
Welcome to Smile Design Dental! Here in Plano, you and your family are always welcomed warmly and treated with respect and honesty. After our team members have taken you on a tour of the office space, you’ll see Dr. Hien Nguyen directly and be able to discuss your dental needs and goals with them. He’ll be happy to help you form a comprehensive treatment plan that’s a perfect fit, whether it includes restorative care for damaged teeth, cosmetic transformations, orthodontic solutions, or other options from our wide variety of available services. We want you to leave feeling better than ever about your smile’s future!
Your initial appointment will consist of a consultation with Dr. Nguyen who will explain your diagnosis and treatment options. If possible, we will try to provide service on the same day. Unfortunately, this is not always an option because an involved medical history or treatment plan will require an evaluation and sometimes a second appointment to provide treatment on another day.
Please assist us by providing the following information at the time of your consultation:
- Any X-rays, if applicable.
- If you are pregnant, please provide us your Medical Doctor number so we can obtain your MD Clearance before your visit
- A list of any medications you are currently taking.
- If you have dental insurance, please bring your dental insurance card with you.
Complete our new patient forms below. These forms could be e-mailed or faxed to you prior to your appointment. This is your registration and medical/dental history.
IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
Instead of having to wait until you arrive at our office, patients can access the forms they’ll need to complete before their first appointment here online. Just follow the links below, print out the documents, and fill in the information whenever you have the time. Then, bring the completed paperwork for our front desk team, and it’ll be time to start your appointment.

INSURANCE WELCOME!
Dr. Nguyen welcomes PPO insurance plans, and our team will be happy to handle all the necessary filing of paperwork so that your available benefits are as maximized as possible. We also accept Medicaid and Medicare for children up to 21 years of age and the elderly, cash, check, and major credit/debit cards.
